Stiff neck, numb hands: managing the desk worker's cervical spine
Male, 43. Desk work, symptoms for 2 years.
C5/6 and C6/7 disc protrusion compressing the nerve root, right arm numb for three months. The approach is release, align, train — relying on passive treatment alone, the odds of coming back in three months are high.
History and prior work-up
Neck and shoulder stiffness and pain for 2 years, with right upper-limb numbness for 3 months over the thumb and index finger (C6 dermatome); no weakness gripping objects; worse after prolonged sitting and looking down, eased by movement, with occasional dizziness.
Cervical MRI showed C5/6 and C6/7 disc protrusion compressing the corresponding nerve roots (radiculopathy); traction and physiotherapy had been tried with short-term benefit; NSAIDs were used. No progressive weakness.
Examination and pattern differentiation
The main pattern is wind-cold-damp bi (painful obstruction) with qi stagnation and blood stasis, combined with liver-kidney insufficiency — desk workers tend to run both stasis and deficiency. Worsening with cold points to wind-cold, fixed stabbing pain to stasis, aching weakness to deficiency. The textbook classification of xiang bi (cervical spondylosis) in Chinese orthopedics and traumatology corresponds.
- Limited cervical range of motion, tight trapezius and levator scapulae, paravertebral tenderness at C5/6; reduced upper-limb sensation, normal strength.
- Tongue pale with a thin white coat; pulse wiry and tight; aversion to cold; a rather high pillow and a habitual head-down posture.
Treatment pathway
- External treatment first: cervical Jiaji points, Fengchi GB20, Tianzhu BL10, Dazhui GV14, Jianjing GB21, Tianzong SI11, Quchi LI11, Waiguan TE5, Houxi SI3, Hegu LI4; tuina to release the shoulder and back muscle groups (heavy techniques are contraindicated in myelopathy — imaging is confirmed first); needle warming or moxibustion at Dazhui may be added. Two to three times weekly.
- Herbal decoction as support: for wind-cold, Gegen Decoction with modifications; for blood stasis, the direction of Shentong Zhuyu Decoction; for liver-kidney insufficiency, the intent of Duhuo Jisheng Decoction or Zuogui Pill. Classical reference: Zhang Zhongjing's Shang Han Lun used Gegen Decoction for the stiff, tight nape and upper back. Dosage is set at the in-person consultation.
- Functional training (essential): deep neck flexor isometrics, scapular stabilization, chin tucks, cervical stretching; 10 minutes per session, once or twice daily — this is what keeps the treatment gains.
- Concurrent management: workstation adjustments (screen height, chair support, standing to move every 45 minutes).

Follow-up and advice
The goals are framed as 'reduce pain intensity, restore neck mobility, reduce recurrence'. Progressive loss of strength, a walking-on-cotton sensation in the legs, or bladder and bowel changes require immediate medical attention (signals of cord compression); no manual therapy in that setting.
