Hurting everywhere with nothing to find on tests: finding the way into fibromyalgia
Female, 40. Symptoms for 4 years, diagnosed by rheumatology.
Widespread pain, fatigue, insomnia, and mood swings woven together. The way in is not 'pain relief' but managing three things at once: sleep, graded exercise, and mood.
History and prior work-up
Widespread muscle pain for 4 years with fatigue, disturbed sleep, morning stiffness, and mood swings. Pain over the neck and shoulders, lower back, and limbs; sleep shallow with frequent waking; marked fatigue; reduced memory and concentration; accompanied by irritable bowel and intermittent headaches.
Thyroid disease, inflammatory rheumatic disease, vitamin D deficiency, and sleep apnea had been excluded. Pregabalin and duloxetine were tried with partial relief but marked side effects; comorbid anxiety and depression, with a psychological assessment already done.
Examination and pattern differentiation
The main pattern is liver constraint with spleen deficiency and qi-blood insufficiency, combined with blood stasis obstructing the collaterals and heart-kidney non-interaction — several systems interwoven. The key points are how strongly the pain couples with mood and sleep, and the distribution pattern of fatigue and aching.
- Aching, migrating pain; multiple tender points; fatigue and mood swings prominent; poor sleep.
- Tongue pale red with a thin white coat; pulse wiry and thready; aversion to cold, easily sweating; average appetite, stool alternating between dry and loose.
Treatment pathway
- External treatment first: Baihui GV20, Yintang, Shenmen HT7, Neiguan PC6, Zusanli ST36, Sanyinjiao SP6, Taichong LR3, Yanglingquan GB34, with Hegu LI4 paired with Taichong (opening the four gates). Technique should be gentle — patients with fibromyalgia are sensitive to strong stimulation, which can worsen symptoms, and this is explained in advance. Twice weekly.
- Herbal decoction as support: for liver constraint with spleen deficiency, the intent of Xiaoyao Powder combined with Ganmai Dazao Decoction; for qi-blood insufficiency, Guipi Decoction; for blood stasis, Shentong Zhuyu Decoction with modifications. Classical reference: Zhang Zhongjing's Jin Gui Yao Lue used Ganmai Dazao Decoction for zang zao (visceral agitation) and Huangqi Guizhi Wuwu Decoction for bi pain from blood deficiency. Dosage is set at the in-person consultation.
- Concurrent management (equally important): graded exercise therapy (starting from very low-intensity walking or water exercise, increasing step by step, avoiding one hard training session), sleep rhythm management, stress and mood management; coordination with rheumatology and psychological care when necessary.

Follow-up and advice
The goals are framed as 'reduce pain and fatigue, improve sleep and daytime function'. The course is long and response slow, so assessment proceeds on 4-week nodes. With marked depression or anxiety, or any thoughts of self-harm, referral to psychiatry or psychological care is required.
